Milestone xcap-diff using publish

This is required to make OpenXCAP independent of the SIP presence agent implementation. Currently the notification for changes in the xcap documents are triggered by a management interface command that is compatible with OpenSIPs only. By sending PUBLISH messages the server can become independent of the PA.
